testing beyond the sandbox

Once I've tested successfully in the sandbox, is it necessary to test on the merchant account? If so what is the proper way to do that, and do I use the test numbers? Will any testing result in fees for the merchant, and how to avoid that, and how to avoid any other issues, like having to void transactions.. etc.

I find the training guides for dev very minimal, can someone fill in the gaps for people who have never done this before?

You can either

a)turn test mode off, test with a real credit card, there will be fee on each transaction, but you can test a transaction to the merchant account.

b)turn test mode on, with a test credit card, all it does it valid the input, not transaction will be create on the merchant account(transactionID will be 0)

if you test with a real credit card with $0.00 or $0.01 in live mode is there a fee?

Yes, there is a transaction fee for requests in your production environment.
