transaction fails/ clears all fields with empty response/reason codes PHP API

Yesterday, everything was working fine, and has been for months.

I changed nothing and today, when someone submits their card info, all fields are cleared and I get no response - almost instantly.

......php?response_reason_code=&response_code=&response_reason_text=

Any thoughts on what's happening (or not happening?)
